Online Adoption Application

Thank you in advance for your application. If you have any questions or problems, please email

Thank you.

NOTE: If you saw MILLIE's listing in the Star-Ledger or, please note that there was a misprint. Millie needs a home with no small children, and if there are older children in the home, they must be calm. Millie should not be in a home with active children. She MUST have a fenced yard and another playful dog. PLEASE DO NOT CALL THE PHONE NUMBER LISTED. It is incorrect. To inquire about MILLIE, please fill out this application or email Thank you!

Your Name
Co-applicant name
Note that we usually adopt only to NJ and eastern PA. If you live outside these areas, please note that we may not be able to adopt to you.
About your home
Rogers' Rescues does NOT require a fenced yard in order to adopt. However, each foster dog has its own individual needs. The foster home will be able to tell you what the dog you are applying for requires based on their experience with that dog.
About your new dog
Include anything that is important to you; breed, size, temperament, age, etc.
If you are unable to adopt immediately (e.g., you have a vacation planned) please tell us, and please understand that we need to open up foster spaces. We try to avoid "holding" dogs whenever possible, because we want to get as many dogs into foster care and adopted as possible. If you're going to need us to hold your dog, we're most likely to be able to accommodate that request if you tell us now. Thanks!
Please check all you feel you cannot handle. Please be honest. If you don't want to deal with any of these issues, please check the box. It is our goal to match you with the dog that best fits your criteria.
Include things such as where the dog will sleep, when the dog will eat, when and how much the dog will exercise, whether you plan to travel with your dog often, and who will make sure that the dog has what he or she needs.
Other pets
List type, breed, age, sex, and approximate weight of all CURRENT pets, and describe their general personalities. Be sure to include whether or not current pets have had interactions with other dogs.
If you are adopting a puppy and he or she is not already spayed/neutered at the time of adoption, you will be responsible for having this surgery done at your own expense by 6 months of age. All adult dogs are altered by our rescue prior to adoption unless they cannot be altered for medical reasons verified by our vet.
Checking the box below gives Rogers' Rescues permission to contact your vet for information regarding your current pets. Please contact your vet immediately and give them your permission to release your information to us so we can verify your current pets are spayed/neutered and up-to-date on routine vaccinations. Many vets will not speak to us without your authorization. Please check the box below to certify that this is your electronic signature, and it is binding on this application. You further certify that the information you have supplied in this application is true. You understand and agree that falsification of any of the information on this application or any information disclosed during the review process is grounds to disqualify your adoption application and nullify all adoption(s) and/or adoption agreements between the applicants and Rogers' Rescues.
